Special Needs Populations Registry Program

The Special Needs Populations Registry Program is confidential and voluntary. This project is a cooperative effort between the City of Olathe Human Relations and Emergency Preparedness. This information will assist Emergency Responders and Emergency Management personnel to identify your unique characteristics that require special needs for the purpose of a safe evacuation or to assist you in the case of a bona fide declared emergency.

To participate in the program, simply print the Special Needs Registry (PDF) and HIPAA Release (PDF), fill them out by hand and mail to:

Please make a copy of the completed forms for your records.

Please note: Both the Special Needs Registry and the HIPAA Release form must be signed and returned to be in effect.
