The Special Needs Populations Registry Program is confidential and voluntary. The program allows residents with special needs the opportunity to provide information to assist Emergency Management in planning to meet the needs of vulnerable populations before, during and after disasters.
To participate in the program, simply print the Special Needs Registry (PDF) and HIPAA Release (PDF), fill them out by hand and mail to:
