Certificate of Compliance Application

A Certificate of Compliance may be filed with the City of Olathe by printing out and submitting the application that may be downloaded below. Additionally, the application process may be started on-line using this secure site.

Based on answers to the questions that are on the on-line form, links will be provided to the forms that will be required to complete the application. These forms will also need to be submitted following the instructions below.

Once printed and completed the application can either be faxed and mailed or simply just mailed. Please note that if an application is submitted via fax, the completed application must still be submitted by mail.

Application Components: Complete those portions that are applicable to your Company, Firm or Agency

• Questionnaire on Personal Practices (5 pages - includes cover letter) - This information is to be submitted with your bid as a Contracting Company, Firm or Agency in accordance with the requirements of the Municipal Code of Affirmative Action/Equal Opportunity and Contract Compliance for the City of Olathe, Kansas. This statement must be notarized.
• Affirmative Action and Equal Employment Opportunity Declaration (1 page) - Must be completed if your Company, Firm or Agency does not have an Affirmative Action Plan.
• Affidavit of Minority And/Or Women's Business Enterprise Joint Venture (2 pages) - To be completed if your Company, Firm or Agency is a minority or women's enterprise joint venture.
• Affidavit of Minority And/Or Women's Business Enterprise (1 page) - To be completed if your Company, Firm or Agency is a minority or women's enterprise.
• Work Force Report Form (1 page) - Complete this form only if your Company, Firm or Agency does NOT submit an EEO Report or any other Federal Report to the Equal Opportunity Commission.
